If you have a large medical file and want a consultation prior to making an appointment, please complete the New Patient Packet of forms and forward them with your medical files. We will schedule you for a 30-minute consultation with Dr. Jerry Hesch for only $125. After reviewing your medical records Jerry will be able to let you know if he will most likely be able to help with your complaint(s).

Chart Review & Consultation
Initial new patient chart review & phone consultation is $125. Payment is due prior to your first visit since the chart review is done prior to your first visit.
