Note: Denver International Airport is 20 mins north of Hesch Institute with many other hotels nearby if that location is preferred.

If you are planning on being treated at the Hesch Institute, the following paperwork below must be filled out via either option.
 

(We accept all major Credit Cards, Check, and HSA/FSA Cards. We don't bill medical insurance companies, but will provide you with a detailed invoice with all required information in order to file a claim with your insurance provider.)

Post-Consult Virtual Follow Ups
Upon request, we can schedule a follow-up phone call or video conference (e.g., Skype, Zoom, FaceTime) at any time after treatment, ideally 1-2 weeks after your last visit. We can also have you send a video of you performing the self-treatment and home exercise program. One option is to upload it on YouTube as a private link and forward the link. Of course, you can contact us at any time.
